CHA Summer Reading Project
View the Summer Reading Project for all students due September 2022. 
*Please note, all incoming Freshmen are required to complete a Summer Reading Project as well. 

 COVID-19 Testing Consent Form
All students are required to provide consent for Covid-19 testing by their scheduled in-person learning day. Please complete this consent form COVID-19 Consent Form and return to CHA on your first in-person learning day. 

Daily Required Health Screening
All students are required to complete the DOE Health Screening every day before entering the building.
You can add a shortcut for the screening to your phone by clicking "Add this tool to your phone" once you click on the link.

CHA Device Request Form
Fill out the form if you need a device to use at home during remote learning. Please note, this request will only be processed if you have NOT already borrowed a device from the school or the DOE. Please contact the school at (718) 736-7320 to schedule an appointment to pickup your device after you have completed the form. If you need a replacement device, you will need to RETURN the device that you already have in order to receive a replacement.
