Please note, this request will only be processed if you have NOT already borrowed a device from the school or the DOE. Please contact the school at (718) 736-7320 to schedule an appointment to pickup your device after you have completed the form. If you need a replacement device, you will need to RETURN the device that you already have in order to receive a replacement.
